State of Illinois Disability Survey

Each State employee is given an opportunity each year to participate in the State online disability survey. The purpose of the survey is twofold;

  1. to give employees an opportunity to identify whether they have disabilities for affirmative action purposes, and
  2. to determine whether emergency evacuation assistance is needed.

Safety personnel will be provided with the names of employees needing evacuation assistance. Employees in opening the survey page have two options, responding to the survey or skipping it. In responding to the survey, employees can voluntarily identify a disability and indicate the need for assistance in regards to emergency evacuation. Survey results are accorded confidentiality.
